How to Book a New Patient Appointment

1. Click the "Patient Portal" button.
2. Choose the appropriate new patient appointment based on age.

NOTE: A credit card is required to be kept on file at the time of booking. Please review the billing policy when adding your card. Please complete your intake paperwork when scheduling your appointment.
